Hazel has over a decade of nursing experience, having earned her Master’s in Nursing (Family Nurse Practitioner) from the University of Texas at Arlington. She is board-certified by the American Academy of Nurse Practitioners and has completed specialized training as a Registered Nurse First Assistant (RNFA). Additionally, she has received aesthetics training from the National Laser Institute. Currently, Hazel divides her time between her role as an Orthopedic First Assistant and managing her Medspa. She is passionate about leveraging her skills and expertise to help others enhance their well-being both inside and out.
Martin Fielder MD

Martin W. Fielder, M.D., is a distinguished obstetrician and gynecologist with a comprehensive medical background. He earned his medical degree from the University of Oklahoma College of Medicine in 1995, after graduating with distinction from the University of Oklahoma in 1991. Dr. Fielder completed his internship at SUNY at Stony Brook, NY, from 1991-1992 and went on to complete his residency in Obstetrics and Gynecology at the University of North Carolina in 1999. He proudly served in the U.S. Navy from 1999 to 2004, where he was stationed at the National Naval Medical Center in Bethesda, MD. Board certified by the American Board of Obstetrics and Gynecology, Dr. Fielder is also a Fellow of the American College of Obstetrics and Gynecology. Since 2013, he has been dedicated to providing exceptional care to his patients in Sulphur Springs.
